Please print these and fill out before your appointment to make your visit as efficient as possible.
You're in great hands
We want every patient to feel at home in our office. This means you should feel welcome, safe, and taken care of. During your initial visit, either Dr. Tim or Dr. Justin will review your dental and medical history. In order to properly diagnose any disease they will examine your teeth and take X-rays of your mouth. After gathering all necessary information, they will explain any issues and help design a treatment plan that works best for you. If there is still time remaining, we typically proceed with a cleaning at the initial appointment as well.
When you contact our office for a new patient appointment you will be mailed a "New Patient" package in the mail. To minimize your wait time at the office please come prepared with all necessary paperwork filled out.
